A RAID log is useful only while it changes decisions
Record each item in plain language, give it one owner, and state the next action. Review the log on a fixed cadence and close entries that no longer affect delivery. A long static register is documentation, not risk management.
Keep the owner and response beside the score
A coloured square without an owner changes nothing. Each important item needs a person responsible for watching it and a concrete response: avoid, reduce, transfer, accept, resolve, validate, or unblock.
Common questions
How is the risk score calculated?
Likelihood from 1 to 5 is multiplied by impact from 1 to 5. Scores below 6 are low, 6 to 14 are medium, and 15 to 25 are high.
These bands are a practical default. If your organisation has an agreed scoring policy, use its thresholds when reviewing the exported register.
What belongs in a RAID log?
Risks are uncertain events that may happen. Assumptions are beliefs that still need validation. Issues are problems happening now. Dependencies are things delivery relies on.
Every material entry should have one owner and a response or next action.
How often should the register be reviewed?
At a cadence that matches how quickly the project changes: weekly is a useful default for active delivery. Review high risks more frequently when their trigger is approaching.
Close obsolete items instead of letting the register grow forever.
Is project information stored or uploaded?
